Contacting Your Insurance
Each insurance plan is different and your insurance company representative will be able to provide you with plan-specific information.
You can contact a customer service representative at your insurance company by calling the (800) number on the back of your insurance card. If you’ve lost your insurance card, your Human Resources department or your school’s health services will also have this contact information.
