Forms of Payment
Michigan Avenue Immediate Care physicians and staff will assist you in meeting your travel needs. We offer a comprehensive listing of vaccines, competitive fees, and accept multiple forms of payment.
We accept the following forms of payment for your travel medicine visit and vaccines at the conclusion of your visit:

Unfortunately, most insurance companies* do not cover travel medicine visits, but we do provide self-pay pricing. Michigan Avenue Immediate Care will provide you with the following documentation should you decide to submit the claim on your own to your insurance carrier:

Medical records
* BCBS (PPO) is currently the ONLY insurance company Michigan Avenue Immediate Care can bill for Travel Medicine visits
• BCBS PPO patients electing to use their insurance are responsible for their Primary Care copayment at the end of their visit
• Patients should be aware of the possibility that claims may not be covered; there are two possible outcomes:
o Claim Denied: We will provide self-pay pricing
o Applied to Deductible: We will be unable to discount the fees
