This is my written request for a doctor’s note stating my inclement health, a reason for the absence from school. Please let me know if you need any more info or details.
Thank you very much for your professional courtesy and swift, decisive treatment. I was impressed by your efficiency and thankful for your care. I plan on turning to you first for medical attention should the need arise in the future.
Again, thank you,
Dear Doctors & Staff at MAIC,
Just a note to thank you for your care for our son a long way from home at art school and also for your introductory letter that came with your bill. This information you sent about who works at Michigan Avenue Immediate Care is helpful to us, particularly being at this distance. I also thought the flier you included was very useful especially for a student possibly on his own for the first time and learning how to handle insurance issues.
While we hope our son will not require your services, we’re grateful to know your names and contact info if he does.
“We really appreciate your follow up, your services, taking good care of our students and being such a close-by resource.”
